For a certified copy of a birth or death entry, from City Hall, the cost is $18.00 plus a business size, self-addressed, stamped envelope (SASE). City Hall needs at least a month and year date as the records are filed chronologically.

Birth or death certificates request may be made by:

Mail — when requesting a birth or death record include the name on certificate, date of birth or death, mother's maiden name, father's name, money order, certified check (made payable to "City of Youngstown"), or charge card. Self addressed stamped envelope.
In person — Hours are Monday - Friday, 8:00 a.m. - 4:00 p.m.
